The ORIGINAL signed copy of the forms must be submitted to the register for final registration. Please include ALL of the pages from the forms.

Please remember to ALLOW MACROS (Aktiver makroer) when opening these forms. For MS Word 2000 and above this means choosing no higher than MEDIUM Security under 'Macro' section of the MS Word 'Tools' menu. The forms may be used either as pre-approval drafts by fax or e-mail, and for final submission by regular mail.
